We are committed to provide you with excellent care by utilizing high-end technology and personalized care. It is our goal to offer outstanding service to all of our patients. This page is intended to provide you with information about what to expect about your visit. Be sure to fill the forms to the best of your knowledge. Your insurance card and a government photo ID will be required at every visit. Our goal is to be your provider of choice.

Getting To Know You:

We would like to help you get the most out of your visit. The time you spend with your physician is very valuable. Listed below, are a few forms to be filled out before your appointment visit.

  1. Complete the Patient Information Sheet.(attach form)
  2. Complete the Personal Medical History. Be sure to include all medications, including over-the-counter medications and supplements. Make sure you let your provider know about any refills you may need.
  3. Review and Sign the Notice of Privacy Practices. (attach form)
  4. You may optionally fill out:
    1. Medical Release Consent Form
    2. Advance Directive Forms. (attach form)

During your First Appointment:

Your First Appointment will be the oppartunity for us to get to know. You will be asked about your medical history and will have a detailed physical exam. Most ofthe time patients are seen in primary care office, for the following three major reasons, which include, sckvisits, chronic disease management (e.g. Diabetes, High BP, High Cholestorel, etc.) and for preventative care, most commonly known as Complete Physicals.

Sick Visists - If you are not feeling well, our main goal is to see you on a priority basis to meet your medical needs. If you dont have a scheduled appointment, you are welcomed to come as a walk-in. For your convenience, we are also open on saturdays 10:00am 01:00pm

Chronic Disease Management - GHP clinic is comitted in providing you with medical care based on approved guidelines in order to prevent the long-term complications of your medical conditions.

Electronic Medical Records

We use electronic medical records to maintain all your health information. We also interface electronically with all major laboratories in this area including QUEST and LABCORP. Laboratories results come automatically into your patient record once they become available. In addition all documents from hospitals, specialists, radiology centers are scanned into your electronic chart.

PRESCRIPTION

  • GOOD HEALTH PHYSICIANS has the capabilities of writing and sending prescriptions to a participating pharmacy electronically instead of using handwritten, faxed notes or calling in prescriptions.

During Check-out, you will also be provided with:

  • Receipt of Payment including your charges, co-pay, deductibles and your next appointment.

What we expect from you:

In order to achieve the best quality care we believe that as a patient you have some responsibilities:

  • You have the responsibility to be accurate and thorough when providing information about your medical history to our medical staff in order to effectively care for you.
  • You have the responsibility to ask for clarification about any aspect of your care which you do not fully understand and to participate in developing mutually agreed upon treatment goals.
  • You have the responsibility to follow the agreed upon plans and instructions for your care.
  • You have the responsibility to notify your health care provider if you notice any change in your health.
  • You have the responsibility to extend reasonable courtesy toward all health care providers during the treatment process.
