How Can I Get An Appointment?

If you are an existing patient [click here]. However, if you are a new patient you will be asked to sign portal access forms and request our office to authorize and enable your account. You can also book online appointments from website or call our office to make an appointment as well.

How Can I Cancel My Appointment?

If you would like to cancel your appointment, call us 24-hours before your scheduled appointment.

If am Sick and Need to See My Doctor Today, Can I Come in as a Walk-In?

Yes. If you are sick and need to see your doctor today, you can come as a walk-in Monday-Friday from 8:30am-5:30pm and Saturdays from 10:00 am.-1pm.

How Can I Request a Medication Refill?

Please call 24-hours ahead to request your medication refills. Also, you can request a refill through our website [click here]. However, all controlled substances require an appointment with the physician for refills.

How Can I Get a Referral?

To get a referral, you must be seen by the physician and discuss the referral that is needed. The referral department will complete the recommendation within 3-5 business days.

When Can I Get My Lab Results?

Lab results will be received by your physician and will be reviewed with you at your follow-up appointment. The patient will be contacted immediately in the event of any abnormal labs. You can also access your results online through our secure patient portal once the physician reviewed the results.

How Can I Get a Copy of My Medical Records?

You can come to the office and request a copy of your medical records. However, according to the ‘Florida Statute 395.3025 Rule 64B8-10.003, Florida Administrative Code’ your charges should not exceed the following:

‘Per Page Fee - $1.00 per page for the first 25 pages then .25 cent per page after. Mailing Fee may charge sales tax actual postage.’ You can also request your medical records via-fax with no fee only if you give authorization to your specialist that needs your records.

How Can I Get More Information About Health Coach?

Our office is developing coordinated health care model in accordance with NCQA guidelines. Please ask at your next appointment who is your health coach. Our goal is to provide quick, comprehensive care. Your health coach will ensure that all of your needs are met.

What Is My Payment and Finance Policy?

All copayments and deductibles are expected to be paid at the time of the visit. We accept all major credit cards, cashier checks, money orders and cash as method of payment. Under state law, your insurance company has 30-days in which to process and pay the claim, request more information, or deny the claim. If the insurance company has not notified us within 90-days of the date serviced, it will be assumed that your insurance coverage is no longer active. At that point, the patient is held fully responsible for payment.

Past Due Accounts:

Patients with outstanding accounts will be required to make payments at the time of appointment. If you are unable to make a mutually agreeable payment arrangement, we will be more than happy to reschedule your appointment. However, accounts with a 90-day past due and failing to honor the agreed-upon payment terms will be sent to a collection agency. If your account is sent to a collection agency, you must pay all past due amount or make agreeable payment terms before subsequent appointments. Additionally, for any financial matters, patients can be dismissed from our practice and will have to seek their health care elsewhere.
